Appendix to Emergency Response Plan

General

The Emergency Response Plan for the A. R. Smith Department of Chemistry and Fermentation Sciences serves as a supplement to the Appalachian State University Emergency Response Plan. The departmental response plan refers to specific responsibilities and duties of faculty and staff as stated in the Chemical Hygiene Plan adopted by Appalachian State University.

Purpose

The purpose of this plan is to outline the organizational structure of the A. R. Smith Department of Chemistry and Fermentation Sciences and to identify duties and responsibilities of faculty and staff members of the department in order to insure the safety and health of University employees, students, and others in the event of a major campus emergency or disaster.

Policy

  1. Departmental faculty and staff, including student employees who assist the Laboratories Manager and/or the Administrative Support Specialist, will become familiar with the elements of the University Basic Emergency Operations Plan to ensure a timely response to any and all possible campus emergency situations.
  2. Departmental faculty and staff, including student employees who assist the Laboratories Manager and/or the Administrative Support Specialist, will become familiar with this supplemental Emergency Response Plan which is designed specifically for the A. R. Smith Department of Chemistry and Fermentation Sciences. Faculty and staff will implement specific responsibilities as noted in the University Plan and in this supplemental Plan.
  3. Departmental faculty and staff, including student employees who assist the Laboratories Manager and/or the Administrative Support Specialist, will become familiar with the Chemical Hygiene Plan adopted by Appalachian State University so as to ensure the safe handling and use of chemical substances in the classrooms, instructional laboratories, research laboratories, and chemical storeroom facilities operated by the department and to ensure the safety of students who enroll in and participate in courses and activities offered by or sponsored by the A. R. Smith Department of Chemistry and Fermentation Sciences.
  4. This departmental plan will be reviewed at least annually by the departmental safety committee and updated as needed under the direction of the Chair of the A. R. Smith Department of Chemistry and Fermentation Sciences.

Notification

  1. A. R. Smith Department of Chemistry and Fermentation Sciences faculty and staff members may be contacted at Ext. 3010 or the specific extension listed in Table I - Contact persons during an emergency below.  The department chair and individual faculty or staff members may also be contacted at home or by cell phone as indicated.
  2. Faculty or staff of the A. R. Smith Department of Chemistry and Fermentation Sciences who observe any emergency or disaster in the classrooms, instructional laboratories, research laboratories, chemical storeroom facilities operated by the department or in elevators or hallways where chemicals are transported, will immediately report this incident to University Police by dialing Ext. 8000 and giving details of the nature, location, and extent of the incident. Faculty or staff who observe and report such incidents will make themselves available for information purposes to the emergency officers who respond to the scene.

  • The Department chair is the primary contact for all flood or fire emergencies. In addition to the chair, the faculty member in charge of the specific area should be contacted as indicated in Table 1 “Contact persons during an emergency”. When the primary contact will be out of town an alternate primary contact will be designated and the Police will be advised of that person’s contact information.
  • In the event that the primary (or alternate primary) contact cannot be reached, the secondary contact is the Chemical Hygiene Officer for the department.
  • The Stockroom and Research Lab inventories are on file in the A. R. Smith Department of Chemistry and Fermentation Sciences office and with the University Police.

Concept of Operations

  1. When notified of a campus emergency or disaster situation involving the classrooms, instructional laboratories, research laboratories, chemical storeroom facilities operated by the department or in elevators or hallways where chemicals are transported, A. R. Smith Department of Chemistry and Fermentation Sciences personnel will cooperate with University Police and emergency services personnel as requested to provide priority protection for the life safety of university employees, students, and visitors and to provide preservation of university property.
  2. When notified of a campus emergency or disaster situation involving the classrooms, instructional laboratories, research laboratories, chemical storeroom facilities operated by the department or in elevators or hallways where chemicals are transported, A. R. Smith Department of Chemistry and Fermentation Sciences personnel will cooperate with University Police and emergency services personnel as requested to determine the nature and extent of the emergency incident.

Responsibilities

  1. In the event of any reported campus emergency or disaster involving the classrooms, instructional laboratories, research laboratories, chemical storeroom facilities operated by the department or in elevators or hallways where chemicals are transported, A. R. Smith Department of Chemistry and Fermentation Sciences faculty and staff will carry out responsibilities as described in the University Basic Emergency Operations Plan and the departmental Emergency Response Plan.
  2. At the conclusion of any reported campus emergency or disaster involving the classrooms, instructional laboratories, research laboratories, chemical storeroom facilities operated by the department or elevators or hallways where chemicals are transported, the Chair of the A. R. Smith Department of Chemistry and Fermentation Sciences and members of the Department Safety Committee will conduct a critique of the incident to determine the adequacy of the responses, the adequacy of the University Basic Emergency Operations Plan and the departmental Emergency Response Plan. Needs for additional training of campus personnel will be identified at this time.
  3. The Chair of the A. R. Smith Department of Chemistry and Fermentation Sciences will participate in an annual review, update, and exercise of the University Basic Emergency Operations Plan.
  4. The Chair of the A. R. Smith Department of Chemistry and Fermentation Sciences and members of the Department Safety Committee will conduct an annual review of the departmental Emergency Response Plan.
